Once you request your preferred day and time of appointment, we will follow up with an email with all intake paperwork. Please complete this paperwork in a timely manner so we may check your insurance benefits before your visit.

Your intake paperwork will be completed in our confidential patient portal, which you can use to communicate with your child’s therapist; make, change, or cancel appointments; and pay your bill.

For insurance purposes, you can add your prescription from your physician one of three ways: add it as a picture on the intake paperwork, bring it with you to your evaluation, or have your referring physician fax it to us.

You will receive a text message reminder the day before your appointment.
